Summary:
The diabetes educator has the responsibility to support and ensure the implementation of education for the diabetic patient mainly in the in-patient setting.
Functions as a resource for diabetes management.
Provides leadership and promotes development of the patient care team for the education phase initiated for the in-patients and out-patients.
Major
Responsibilities:
Responsible for the coordination of education of patients, significant others and community related to diabetes self management as part of the continuum of care.
Develop, implement, and document teaching plan, educational interventions, and follow-up plan based on the learning needs assessment for patients, families, and/or significant others.
Collaborate with patients, families, and/or significant others in establishing mutual learning goals and behavior change goals.
Document evaluation of outcomes, and revises interventions and follow-up plan for patients, families, and/or significant others.
Access resources to facilitate discharge planning and appropriate follow-up.
Implement and coordinate outpatient education and follow-up needs.
Assure education is based on current standards of practice and input from physicians.
Participate in planning and conducting community service projects and programs.
Communicate education and follow-up recommendations to physicians and other health care providers within the multidisciplinary team.
Initiate a program that will meet the National Diabetes Advisory Board (NDAB) Standards in order to obtain the Education Recognition Program (ERP) certificate from the American Diabetes Association.
Identify education resources needed to achieve objectives for target population, including adequate space, personnel, budget and instructional materials.
Oversee standing advisory committee which consists of a physician, nurse educator, dietitian, behavioral science expert, consumer, and a community representative to oversee the program.
Ensure content areas as outlined by NDAB for instruction.
Use instructional methods and materials appropriate for the target population Follow all other standards as set forth by the NDAB Assess, Plan, Implement and Follow-up with participants as recommended by NDAB Maintain written policy affirming education as integral component of diabetes care Maintain and provide staff development for a core of patient education representatives whose function is to assist with unit patient education, materials/program assessment, planning, and development, evaluation and revision.
Coordinate patient educational activities with all health care disciplines involving patient education.
Identify patient education needs.
Prioritize the development of patient education materials as indicated by needs assessment.
Advise and participate on patient education committees for specific patient populations.
Coordinate the standardization of patient education tools for patient education in patient care departments.
Provide periodic evaluation of patient education efforts as necessary.
Participate in Nursing Orientation.
Develop and maintain inventory of patient education materials.
Meet with sales representatives regarding products and supplies for patient education.
Serve as a resource in diabetes management for the healthcare team.
Serve as a resource person and mentor to staff related to clinical knowledge of diabetes management.
Act as a resource to other departments and disciplines within hospital and community.
Participate in formal or informal educational programs regarding diabetes related topics.
Maintain knowledge of current developments, trends and techniques in diabetes management and education and makes recommendations regarding use in diabetes program.
Incorporate activities as appropriate to ensure positive patient outcomes.
Gather and analyze data related to quality outcomes for the diabetes education program.
Participate in diabetes advisory committee and other committees as appropriate.
Recognize the opportunity to integrate research findings into nursing care and education of patients with diabetes.
Participate in professional organizations related to the subspecialty in order to promote and maintain current standards of care and patient education.
Education/Experience Required:
Bachelor of Science in Nursing, Human Nutrition or related field.
3-5 years experience in clinical specialty.
Knowledge, Skills & Abilities Required:
Strong leadership skills.
Ability to communicate and work cohesively with diverse groups.
Excellent organizational and planning skills.
Specialty certification in Diabetes (CDE) required.
Current license in the State of Illinois as a registered nurse required.
CPR Physical Requirements and Working Conditions:

If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015.
Ability to lift up to 35 pounds without assistance.
For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
Unique patient lifting/movement situations will be assessed on a case-by-case basis.
